1.  Specifically describe the problem.  Include all details.

2.  Problem Category:

Speech recognition related
Other Portable Terminal or speech system issue
PC-based application issue
Other issue

3.  When did the problem first occur?

4.  Is the problem constant or intermittent?   If intermittent, how often does it occur (several times a month, once a week, etc.)

5.  Describe system operation before the problem?

6.  Is this problem specific to this voice terminal, voice station, headset or other component?  Or does it happen with all units?

7.  Does this problem occur when using a charged, good battery (if applicable)?

8.  What things changed recently close to the time this problem occurred?

  • New Voice System hardware deployed?
  • New types of data (new part numbers, new defects, new descriptions)?
  • New user, new person using system?
  • PC changes?  New hardware or software installed on PC?  LAN/Network changes?

9.  Any other possibly-related events?  Power outage?  Network problems?
